Try using a clean fluxcp or a newly installed fluxcp. It happens to me and I try to make a new installation and it works.

I think its similar to what I experience when I use old fluxCP. Try using a fresh fluxcp then try to install the addon again

In order to add vote you must logged as an admin. Since the default allowed Account for Level for Add/Edit/Delete function is 99.

Base on this error it seems your FluxCP has no rights in CREATE TEMPORARY TABLE. You should grant your FluxCP a CREATE TEMPORARY TABLE in your MySQL.
