When you're in a Client & Patcher support section, you should always mention what your client date is / what patcher you are using. Moving on, later 2012 clients requires you to add your custom item into your System/iteminfo.lua[lub] as well. It looks like you have your sprites working correctly too. Are you sure you have the item icons in the proper folder? Also, making sure all your customs are named the same.. if there's one spelling error, it won't work.
