as what the error said..the url you requested cannot be found...try to check your code or the path that you specify in your code if it has an error or if you put the correct path...

If the addon is on the right directory/path, try to rename the folder of your theme to default and edit as well your application.php and change the old theme name to default. I have experienced that kind of error as well and this is the simplest solution I have found so far.
