Help Coding Registration Web Template

Someone Here can help me coding this to connect to my database for free? but if it takes a lot of time or its too hard I understand that you guys can't do it for free, because I dont know if its simple or no to code that. But if exists a tutorial on how to code that, I would like to see, because I didnt found nothing
